Chronic diseases are characterised by complex causes, multiple risk factors, a long latency period and course of illness, and impairment.
Up until the 20th century infectious disease and injury dominated health issues. During the 20th century chronic illnesses such as heart disease and diabetes have impacted on the more affluent societies. Chronic diseases are now reaching epidemic proportions in countries such as Australia.
In Australia it is estimated that chronic diseases contribute to 80% of the total cost burden of disease and more than 50% of deaths.
The government has therefore asked doctors in general practice to increase their efforts in improving the health outcomes of patients who have chronic diseases. The government also recognises that chronic illness relates, in many instances, to lifestyle issues (i.e. diet, exercise, etc) so doctors and patients need to work together in improving patients health.
KRS Health doctors endorse the initiatives of the government in this area and when a KRS Health doctor identifies a patient with a chronic disease, he/she may ask the patient to enter into a GP Management Plan and a Team Care Arrangement (linking the patient to an allied health professional or specialist) to help them manage their disease better.
If your doctor enters into a GP Management Plan with you, he/she is asking you to recognise your contribution to improving your health. This may mean visiting a diabetes educator, dietitian, physiotherapist or other allied health professional who may give you advice and assistance in managing your health. On the other hand it may be an agreement between you and your GP.
After 3 months, your doctor will ask you to return to see how you are progressing against the recommendations of the management plan and specialist/allied health professions.
Doctors of KRS Health see the need for a viable after hours service in Wagga to ensure patients have the opportunity to see a doctor outside of normal working hours.
KRS Health patients can see a doctor after hours by ringing the Wagga After Hours Service on 69310900 after hours. The after hours service runs from Piercy Place, 185 Morgan Street, Wagga Wagga.
Patients can choose alternative sites to visit doctors. They can choose to see their doctor either at Fernleigh Road or Kooringal Road.
KRS Health doctors also practice from the Community Centre in Tarcutta 2 days per week.
KRS Health is considering setting up a practice at Forrest Hill to bring a much needed service to this suburb of Wagga. We would be interested in the comments of residents of Forrest Hill about this initiative.
KRS Health doctors are supporters of education and training of new doctors. Senior doctors are closely involved with training institutions and provide training opportunities to student doctors and those who wish to become accredited general practitioners. By doing this they are ensuring doctors in Wagga in the future.
KRS Health doctors have visiting rights (can admit patients) at both the Wagga Wagga Base Hospital and the Calvary Hospital. They also visit patients in aged care facilities.
